Waiver and Release of Liability

    ​I, the undersigned, wish to participate as a volunteer in the park cleanup event organized by The Plastic Revelation (hereinafter referred to as the "Organization").
    I understand that my participation in this event is voluntary and that it involves physical activities, which may include lifting, bending, pulling, walking on uneven terrain, and other tasks associated with park cleanup activities. I acknowledge that these activities involve inherent risks, including but not limited to, the risk of injury, illness, or damage to personal property.
    In consideration of being allowed to participate in this event, I hereby agree to the following:
    1. Assumption of Risk: I assume all risks associated with my participation in this event, whether known or unknown to me at this time.
    2. Release and Waiver: I release, discharge, and hold harmless the Organization, its directors, officers, employees, agents, volunteers, and any other participants from any and all claims, actions, or liabilities of any kind arising out of or related to my participation in this event. This includes, but is not limited to, claims related to personal injury, illness, or property damage.
    3. Medical Treatment: I consent to receive medical treatment deemed advisable in the event of injury, accident, or illness during my participation in this event. I release the Organization and all persons associated with providing such medical treatment from any liability for any injury or damage that may result.
    4. Photographic Release: I grant the Organization permission to use my likeness in photographs, videos, or other digital media taken or made during the event for use in promotional materials, publications, or on the Organization’s website without payment or consideration.
    5. Compliance with Rules: I agree to comply with all instructions, rules, and safety procedures provided by the Organization or its representatives during the event.
    6. Age Confirmation: I affirm that I am at least 18 years of age. If I am under 18, my parent or legal guardian has reviewed and signed this waiver on my behalf.
    7. Electronic Signature Agreement: By typing my name below, I am signing this Waiver and Release of Liability electronically. I agree that my electronic signature is the legal equivalent of my manual signature on this document.
